Services

Residential, out-patient  and community based services are developed on the principles of Compassion, Teamwork, Integrity and Excellence. Services are organized to examine and provide practice and evidence based care, to achieve person centered value outcomes, facilitate treatment adherence and compliance consistent with practice and evidence based models of care.  The NYS Office of Mental Retardation and Developmental Disabilities and the New York State Office of Alcoholism and Substance Abuse Services license CRR and provide funding as well as the NYS Department of Health for service provision. The Center for Rapid Recovery, culturally competent continuum of care model includes:

 

Non-Medicaid Service Coordination The aim of the program is to assist residents of Long Island with locating services and supports for persons with developmental disabilities. Services include assessment of benefit eligibility, planning and coordination, linkage and referral, follow-up, monitoring and advocacy.

 

Day Habilitation Program (DAHAB) The Dayhab program is a pre-vocational and social skills training environment which provides opportunities for persons with developmental dissabilities to acquire community integration and job readiness skills.  Individuals with a diagnosis of mild to moderate developmental disabilities are eligible to apply.

 

Mobile Intervention and Outreach Program (MIOP) -The aim of the MIOP program is to increase awareness about service and supports available to families with individuals diagnosed with developmental disabilities. Public awareness campaigns and outreach is targeted but not limited to communities of colour in Long Island.

 

Family Education and Training - At home or off site educational training and supports is provided to parents and other family members of persons with developmental disabilities. Nassau and Suffolk County residents with family members diagnosed with developmental disabilities are eligible to receive services.

 

At Home Residential Habilitation Program (RESHAB) - At home services are directed toward acquiring, and improving the individual's self-help, socialization and adaptive skills necessary to reside successfully at home. Services and supports are available to families who are approved for waiver services and reside in Nassau /Suffolk Counties.

 

Individualized Residential Alternative (IRA) - The IRA program is a certified group residence with 24 hour staffing support. Activities are formulated to stress individual abilities as well as to provide assistive training in core areas of life including: training in personal care, development in recreation, socialization, medication maintenance, and community integration. Persons with a diagnosed with developmental disabilities in need of residential care services must meet the Long Island Development Disabilities Office eligibility guidelines.

 

Medicaid Service Coordination (MSC) - This service entails planning for and obtaining services and supports for persons with developmental disabilities. Services include assessment of service needs, service planning and coordination, linkage and referral, follow-up, monitoring and advocacy. Regular contact is maintained with the individual, primary care giver and service provider to assist with the planning for and obtaining services and supports. Persons with a diagnosed with developmental disabilities ages 18 and above who are in need of case management services must meet Medicaid eligibility and residency guidelines.

 

Chemical Dependency Illness in Recovery Program -The over-arching goal of the outpatient chemical dependency program is to provide culturally and linguistically competent substance abuse treatment in a manner compatible with the cultural beliefs, practices and language competencies of the client. The primary objective of the program is the promotion and maintenance of abstinence from alcohol and other mood-altering drugs. Nassau County residents ages 14 and above with a diagnosis of chemical dependence is eligible to apply. Hours of operation: M/ W/ 9:00- 8:00pm Tues/Thurs/Fri 9:00-5:00pm.

 

Higher Ground Program -The Higher Ground program offers counseling based on principles of faith in combination with HIV/AIDS and chemical dependence prevention education, to increase positive health outcomes for participants of the program. Residents of Nassau County ages 18 and above with a diagnosis of HIV/AIDS or at high risk for HIV including the partners and families of such persons is eligible to receive care.
